SF (Short Form)-8 Health Survey
Source: Washington University in St. Louis (WUSTL)Date Published: 2001Format: TextAnnotation: This resource is an eight-item survey with a single question for each of the eight domains of health: physical functioning, role limitations due to physical health, bodily pain, general health perceptions, vitality, social functioning, role limitations due to emotional problems, and mental health. This measure is a multi-purpose, short-form health survey of functional health and well-being scores, as well as psychometrically-based physical and mental health summary measures and preference-based health utility index. It is a generic measure, as opposed to one that targets a specific age, disease, or treatment group. The SF-8 represents a shorter version of the original SF-36. This resource was identified by the NIH Disaster Research Response Program (DR2) for researchers looking for pre- and post-disaster data collection instruments.
