  1. Disabilities of the Arm, Shoulder and Hand (DASH)

    Annotation: The Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire is designed to assess physical function and symptoms in people with any of several musculoskeletal disorders of the upper limb. It was developed jointly by the Institute for Work and Health and the American Academy of Orthopedic Surgeons. The tool gives clinicians and researchers the advantage of having a single, reliable instrument that can be used to examine any or all joints in the upper extremity. The questionnaire records symptoms as well as activity performance. Two optional, four-item modules are included in the DASH. The first assesses function in the upper limbs of workers whose daily living requires a high level of physical activity; the second is designed to assess function in people involved in athletics or the performing arts. This resource was identified by the NIH Disaster Research Response Program (DR2) for researchers looking for pre- and post-disaster data collection instruments.

    The DASH Outcome Measure and the Quick DASH are free and publicly available; they are owned by the Institute for Work and Health (IWH), which also holds the copyright on both measures.

    Use of these instruments without charge is limited to clinicians using them only for treatment or assessment of a patient, to researchers using them only for non-commercially related research, and to other not-for-profit users. The instruments may not be sold or incorporated into a product to be sold by anyone. The instruments may not under any circumstances be changed in any way as even minor changes may alter performance. Any other use requires advance written permission from the Institute for Work and Health and requires strict compliance with all conditions attached to such permission, including payment in some cases.
