  1. ISARIC Global COVID-19 Follow Up Study Protocol

    Annotation: The ISARIC COVID-19 Long term protocol & Case Report Form (CRF) survey is designed to follow up adults and children with COVID-19 over time. It can be used for one off or serial follow up of patients post-hospital discharge and for people who were not hospitalized. It is designed to enable patient self-assessment or clinical led assessment, via post, telephone, online or in-clinic visit for wide dissemination and inclusion using limited resources. It can be combined with sampling and further diagnostics.

    Annotation: The case report form for COVID-19 is intended to provide member states with a standardized approach to collect clinical data in order to better understand the natural history of disease. There are two Clinical Data Collection COVID-19 Case Report Forms (CRFs) forms available: CORE and RAPID.

    CORE CRF is a more comprehensive form; with more variables to understand the spectrum of disease and management, especially on pathogen testing, critical care interventions, and medications. It is for use where resources allow the collection of more data and ideal for sites who will use the ISARIC Data Platform online data entry or for those who develop their own (locally-hosted) data capture systems.

    The RAPID CRF form includes fewer variables than the CORE form. Data are focused on specific risks, treatments, and variables identified as priorities. It is for use where data collection resources are limited, and ideal for sites who will use the ISARIC Data Platform online data entry because new (optional) data modules will be incorporated as they become available.
